Pathogenesis of Rheumatoid Arthritis|High‑Impact Literature Schematic Interpretation
🔍 Mechanisms in this paper
The synovial microenvironment of rheumatoid‑arthritis shapes two functionally distinct Treg subsets. Macrophages express membrane‑bound TNF‑α to up‑regulate glycolysis via TNFR2 signaling and sustain the suppressive function of CD25ᵏⁱCXCR6ᵖᵒˢ Tregs. Synovial fibroblasts produce cortisol through HSD11B1, driving naive Tregs to differentiate into CD25ˡᵒAREGᵖᵒˢ Tregs. This subset exhibits repressed glycolysis and impaired suppressive capacity, and secretes AREG to activate synovial fibroblasts and trigger pro‑inflammatory programs. TNFR2 signaling can counteract cortisol‑induced Treg functional damage.
💡 Innovations & Breakthroughs
Multiple single‑cell datasets combined with TCR‑seq were applied to dissect in‑situ human synovial Tregs and distinguish the two Treg cell states. It verified that signals derived from distinct stromal and immune cells separately govern the differentiation of the two Treg subsets. It uncovered the reprogramming effect of glucocorticoid signaling on human Tregs, and confirmed that such Treg subtype signatures are conserved across multiple inflammatory arthritic disorders.
🧠 In‑depth Insights
Local tissue microenvironmental signals, rather than intrinsic cellular properties alone, dominate Treg differentiation and functional status. Treg precursors of distinct origins display differential responsiveness to external stimuli. Diverse cellular components within synovium mediate completely opposite immunoregulatory outcomes.
