High-score Literature Analysis of OA Synovial Fluid Proteomics
🔍 Mechanism of this paper
Based on the large‑scale STEpUP‑OA cohort, this study performed unbiased proteomic analysis on synovial fluid samples from 1361 patients with knee osteoarthritis. Cluster analysis indicates that knee osteoarthritis has no mutually‑independent molecular endotypes; instead, it constitutes a continuously‑changing biological spectrum, where epithelial‑mesenchymal transition acts as the core pathway driving molecular disease variation. Pathways including complement, coagulation and angiogenesis are regulated by clinical features such as obesity and biological sex. Poor reproducibility of pain‑related protein signals suggests that pain is interfered with by multiple extra‑articular factors.
💡 Innovation & Breakthrough
This study adopted a large‑scale synovial‑fluid proteomic cohort of over one thousand subjects with discovery‑validation set design to systematically test the hypothesis of osteoarthritis molecular endotypes. It overturns the previous view that osteoarthritis comprises multiple discrete molecular subtypes and proposes that osteoarthritis represents a continuous biological process. It identifies that obesity and biological sex alter the activation levels of complement and coagulation pathways, and distinguishes molecular‑signal differences between structural damage and pain, providing a basis for subject stratification in clinical trials.
🧠 Key Insights
At the molecular level, osteoarthritis is not a collection of independent diseases but a continuous spectrum. For the same disease, different clinical risk factors can modify pathway‑activation patterns. Synovial‑fluid proteins better reflect local intra‑articular pathological changes; nevertheless, mining pain‑related molecular biomarkers remains challenging, and confounding factors of extra‑articular origins should be taken into consideration.
